There are four different claim forms. Please be sure that you select the
correct claim form for your situation. If you are unsure of whether you are a
potential Class Member or which claim form applies to you, please read the “Q
& A” section of this website.

If an annual pay increase went into effect during the period of your unused
annual leave (projecting forward from the date of retirement, separation, or
death the number of days of unused annual leave), and you were a GS or a FWS
employee, or you were a Foreign Service Officer (FO or FP), or an, Administrative Law Judge or Board
of Contract Appeals Judge, click here.

If an annual pay increase went into effect during the period of your unused
annual leave (projecting forward from the date of retirement, separation, or
death the number of days of unused annual leave), and you were a member of the
Senior Executive Service (SES) or the Senior Foreign Service (SFS), click
here.

If you received a Foreign Post Allowance for housing immediately before you
retired or separated in the foreign area, click here.

If you regularly worked at least one Sunday in each of four consecutive pay
periods immediately before you retired or separated, BUT the lump-sum payment
did not include the Sunday premium pay you would have earned if you would have
remained a Federal employee for the period of your unused annual leave instead
of retiring or separating, click
